Services

Fixed scope, fixed fee, a written answer.

Each engagement is fixed in scope and fee, delivered virtually, and ends in a written deliverable your physician lead, practice manager, and finance lead can act on. Never a share of your Medicare payments. No patient data.

Services

Nine fixed-scope engagements in five areas, in the order a network needs them.

01 · Value

What is the diabetes panel worth, and on which rail?

A written go, wait, or no-go on ACCESS, the revenue compared with what you bill today, and the gaps to close first.

  • Fit and readiness assessmentfixed fee · 3 weeks
  • Diabetes program designfixed fee
02 · Ingestion

Can your systems take the between-visit data in?

What your EHR, device exports, and patient-access API deliver against US Core 6.1.0 today, and what a sprint would add.

  • Ingestion readiness reviewfixed fee
  • Device and lab data to FHIR sprintby scope
03 · Devices

Can your patients participate?

Which patients already own a connected meter, cuff, scale, or wearable, which need one, and what it costs to equip the rest.

  • Patient device readiness assessmentfixed fee
04 · Reporting

Will your reports pass, and keep passing?

Your bundles through the official HL7 validator; a fix list in priority order; one clean re-run; a monthly retainer under which we re-run your bundles ourselves after each CMS guide revision and send you what changed.

  • Reporting conformance checkfixed fee · 10 days
  • Guide-revision retainermonthly · professional services
05 · Model

What does it look like with your numbers?

The model on this site rebuilt on your panel and contracts, as a tool your finance lead can run. On request, a projection for a board or a budget.

  • Business analytics toolfixed fee
  • Revenue projectionby scope

Founding-client rate: 40 percent off the assessment, program design, and sprint for the first three clients, in return for a case study built on counts you approve and permission to name you as a reference. What you tell a reference is yours to say, good or bad, and wherever we quote a founding client we say the engagement came at the founding rate. The conformance check is never discounted. Projections are management tools, not investor-facing financial reporting.

This is for you if
  • You are a Medicare Part B practice or network with patients who have type 2 diabetes, prediabetes, high blood pressure, kidney disease, heart disease, or obesity, monitored at home or not.
  • You want to know whether a monitoring program is worth starting. The answer may be wait, and we will say so.
  • You are an ACCESS participant and the outcome reports are still a worry.
  • You hold a Medicare Advantage contract with quality terms and want the diabetes and blood pressure measures to move.
This is not for you if
  • You have no Medicare patients with these conditions. We will say so in the first ten minutes.
  • You want someone to run your clinic’s workflow or touch patient records. We do neither.
Questions an administrator asks

Before the ten minutes.

Which record systems do you work with?

Any certified system. The conformance check reads your report bundles, not your record system. We have no vendor relationships and sell no software.

What do we have to send you for the conformance check?

Three to five sample bundles built from test records, or your vendor’s export format if nobody builds bundles yet. No patient data, so no business associate agreement.

Our vendor says the exports are already FHIR compliant. Why check?

“FHIR compliant” usually means valid FHIR, not valid against the ACCESS guide and US Core 6.1.0 as CMS runs the validator. The check settles it in ten business days. If the exports pass, you have the receipt.

Who on our side has to be involved?

You, for ten minutes. Then about ten minutes each from billing, the quality lead, and your monitoring vendor. For the check, whoever can export sample bundles.

Is this billing or legal advice?

No. It is operational and technical review. We will say when a question belongs to your billing company, your counsel, or your compliance officer.

How is the fee set?

Flat, or a fixed amount per patient, quoted in writing before work starts. Never a share of any Medicare payment.

Can our AI assistant use your tools?

Not yet. Every deliverable comes as a written report plus machine-readable files (FHIR resources and validator logs) that your systems and your assistant can read today. If you want your assistant to run our checks directly, say so on the ten-minute call and we will tell you what is possible for your engagement.

How it goes

Ten minutes, no slides, no decision.

  1. Ten minutes by phone. Fit, one clinical question, who should join next.
  2. A counts-only questionnaire. About 30 minutes of your team’s time, no patient records.
  3. A 30-minute intake with the people who hold the answers.
  4. A written proposal with the scope, the deadline, and the fee, before any work starts.
